In recent years and months, peanut allergies in children have been in the news frequently, as scientists reveal new insights into why more and more children are developing them and what can be done to avoid them. However, until now, few have studied the connection between peanut allergy and childhood asthma.
A new study has shown that many children who have asthma have a sensitivity to peanuts, but did not know it. Conducted by researchers in the U.S., the study specifically looked at pediatric asthma patients at a pediatric pulmonary clinic.
The study will be presented at the ATS 2015 International Conference.
"Many of the respiratory symptoms of peanut allergy can mirror those of an asthma attack, and vice versa. Examples of those symptoms include shortness of breath, wheezing and coughing," said study lead author Robert Cohn, MD, MBA. "This study aimed to evaluate the proportion of asthmatic children who also demonstrated a sensitivity to peanuts."
For the study, the researchers looked at 1,517 children from the pediatric pulmonary clinic at Mercy Children's Hospital in Toledo, Ohio. They determined if the children's charts had a documented peanut allergy and if they had undergone a blood test for antibodies demonstrating a potential reaction to peanuts, known as IgE. Children were considered positive if they had a documented history of peanut allergy or a specific IgE blood test that showed a level higher than normal.
What they found was that of the 1,517 charts that were reviewed, 163, or about 11%, had a documented history of peanut allergy. Nearly 44% (665) had specific IgE testing at some point to test for peanut allergy. Out of that group, 148, or approximately 22%, had a positive test to peanut sensitivity. However, more than half of these children and their families did not suspect there was any sensitivity to peanuts. The prevalence of positive tests varied across age groups but the prevalence of known peanut allergy was strikingly similar across age groups.
"This study demonstrates children with asthma might benefit from a test for peanut sensitivity, especially when control of wheezing and coughing is difficult to achieve. If a physician is having this problem, or if a parent notices it in his or her asthmatic child, they should consider testing, even if they believe their child is not sensitive to peanuts," said Dr. Cohn. "There should be continued investigation to learn more about the connection between asthmatic children and peanut sensitivity."

Children who develop asthma in Toronto are more likely to have been born in a neighbourhood that has a high level of traffic-related air pollution, new research suggests.
Researchers identified a number of "clusters" where higher numbers of children who developed asthma were born, including Parkdale-Little Portugal, west of downtown Toronto; the neighbourhood where the Don Valley Parkway connects to the westbound Gardiner Expressway; and parts of Scarborough, including where the Don Valley Parkway and Highway 401 intersect and around Cliffside-Scarborough Village.
"In this study, high clusters of atopic asthma [asthma related to allergies] were found in children who were born in parts of the southwest, south and northeast of Toronto," said lead author Dr. Ketan Shankardass, a social epidemiologist with the Centre for Research on Inner City Health of St. Michael's Hospital. "Such clusters support the notion that early life factors at the neighbourhood level are relevant to the development of childhood asthma.''
Shankardass said 70 percent of the children involved in the study had moved from their birth neighbourhood, further suggesting the air pollution during pregnancy and shortly after birth was related to developing asthma later in childhood.
While exposure to traffic-related air pollution helped explain some of these clusters, Shankardass said air pollution isn't necessarily acting alone in causing childhood asthma, or else they would have found similar clusters along all the main thoroughfares in Toronto.
His study was published in the journal Health & Place.
Shankardass said other risk factors that could be associated with childhood asthma include the relatively low socioeconomic status in Parkdale-Little Portugal and Scarborough, which have average median incomes of $51,767 and $52,944, respectively, compared to $65,047 in the rest of the greater Toronto area.
In addition, there may be persistent air pollution in Scarborough from sources other than traffic, such as factories. In 2006, the average area of industrial land-use in Scarborough (62,715 square metres) was far higher than the rest of the GTA.
The Parkdale and Little Portugal neighbourhoods also contain some of the city's oldest housing stock (the average percentage of pre-1946 housing in Parkdale-Little Portugal is 55 percent, compared to 4 percent in Scarborough and 19 percent in the rest of the GTA). That includes many large homes that have been converted into small, crowded apartment buildings and that are poorly maintained, meaning there might be indoor environmental hazards that were not completely accounted for in the study, such as cockroaches and mold.

Bariatric surgery appears to cut risks for serious asthma-related events

A study led by Massachusetts General Hospital (MGH) investigators suggests that bariatric surgery can significantly reduce the risk of asthma attacks -- also called exacerbations -- in obese patients with asthma. Their report, published online in the Journal of Allergy and Clinical Immunology, is the first to find that significant weight reduction can reduce serious asthma-associated events.
"We found that, in obese patients with asthma, the risk of emergency department visits and hospitalizations for asthma exacerbations decreased by half in the two years after bariatric surgery," says Kohei Hasegawa, MD, MPH, MGH Department of Emergency Medicine, the lead author of the study. "Although previous studies of non-surgical weight loss interventions failed to show consistent results regarding asthma risks, our result strongly suggests that the kind of significant weight loss that often results from bariatric surgery can reduce adverse asthma events."
Both obesity and asthma are serious public health problems at historically high levels in the U.S., the authors note, and many researchers have associated obesity with the development of asthma and with an increased risk for asthma exacerbations. While previous studies investigating whether weight loss could reduce asthma risks showed little or no benefit, participants in those studies lost only modest amounts of weight. The current study was designed to investigate whether bariatric surgery -- regarded as the most effective option for morbidly obese patients -- might have a greater effect on asthma-associated risks.
Using available databases reflecting the utilization of health services in California, Florida and Nebraska -- all three of which give access to deidentified information on individual patients -- the research team identified 2,261 obese patients with asthma who underwent bariatric surgery from 2007 to 2009 and for whom information covering the two years before and after their surgery was available. This design, in which participants essentially act as their own controls, reduces the need to control for additional factors -- such as age, gender, genetic background and physical activity -- that might bias the results.
The analysis showed that, during the two years prior to surgery, around 22 percent of the studied patients had at least one emergency department (ED) visit or hospitalization in each one-year period. In the two years after surgery, only 11 percent needed an ED visit or hospital admission in each year. Looking at hospitalization alone showed an even greater risk reduction, from around 7 percent per year to less than 3 percent. A comparison with patients who had other types of abdominal surgery showed that non-bariatric procedures had no impact on asthma exacerbation risk.
While the mechanism by which a significant weight loss can reduce asthma-associated risks is unknown, studies have linked obesity to increased inflammation, higher prevalence of gastroesophageal reflux disease, and physical changes in the airway -- all of which could contribute to asthma severity. Hasegawa notes that a reduction or reversal of these mechanisms by bariatric surgery is plausible.
"The databases we had access to did not include the actual amount of weight lost by these patients, but it is well documented that bariatric surgery results in substantial weight loss, averaging around 35 percent of presurgical weight," he says. "While we can't currently say how much weight loss would be needed to reduce asthma risks, previous studies of non-surgical interventions indicate that modest weight loss is not enough."
"Bariatric surgery is a costly procedure that carries its own risks, factors that may offset the benefits regarding the risk of asthma exacerbation for some patients," Hasegawa adds. "To decrease asthma-related adverse events in the millions of obese individuals with asthma, we probably will need to develop safe, effective non-surgical approaches to achieve major weight loss."
